Database error

On the placement test page, if I select INTERMEDIO (INTERMEDIATE) and then fill in the form, the test comes up. But if I select either PRINCIPIANTE (BEGINNER) or AVANZATO (ADVANCED), I get an error.

Why 1 option would work and the others not?

12/21/18__ CORRECTION: Now none of the tests are coming up. It shows the shortcode like WPPROQUIZ 1 or others… It’s almost like if the WP-Pro_quiz plugin is not running or it is blocked by something. I can find backups of the plugin folder, but not an actual backup (report) that I did from Wp-Pro-Quiz. Suggestions anybody?
