Deeplink Juggernaut doesn't grab all keywords it could

I have an problem with the Deeplink Juggernaut. It works great in most cases but in some posts it doesn’t grab all keywords it should. And also in some posts it doesn’t grab the first occurence of the keyword but the fifth or something. Very strange.
As an example you can see it here: Test
There are several occurences of the word “Leuk?mie” but the only one that’s linked is located near the footer of the page.
Also there are several occurences of the keyword “Aplastische An?mie” but none is linked although a post with the same name exists.Any ideas what’s wrong or what I possibly do wrong?
