    What is the code used to display the image?

    The plugin works with the buildin functions the_post_thumbnail() and get_the_post_thumbnail()

    I can’t see the code of your theme.But can you find out the code that is around <div class="post-thumb">

    Do you have specific pages that should show the default image? Because now on all posts I see thumbs.
