Delivery Date and Time extension?

Hello,
I am asking myself if there is an extension that allows the customer to pick not only the date, but also the time for the delivery?

What I want to do is:

1. I want to select days that are available for delivery.
2. I want to define different time slots with different prices that are available for delivery.

I found this extension, but this one lacks the possibility to define timely windows with additional prices:

Anybody has a suggestion?
