Desk Mess messes up forms

Hi Messy Desk Martes!

I embedded a form from Mailchimp; the form works but the appearance is distorted and ugly.

I used a shortcode form from Optincat and the form looks beautiful, but does not work. Click “submit” and the page just refreshes or redirects to a 404 error.

I tried another theme and both forms work fine. I hope you can tell me there is a workaround and that I don’t have to consider changing themes right now. I am open to creating a special page that would avoid the problem is that is possible, especially within my limited skill set.
