I hard coded the content within the plugin, so could walk you through it if you need me to. I just used underscores theme and created a page, inserted the shortcode. Then added all the html elements to the plugin as the page changes based on the form submission. Then did all the styling using custom css and gridism responsive grid.

Awesome. Probably would tackle it myself but time is limited.

@clritym , let me take advantage of this discussion to ask you something. I have been facing some troubles. When an email is placed in the email input section, Instead of redirecting to another page to show the thank you template, it appears the thank you page inside of my form page.
