display chart by field value

  • Hwo can i display chart by form field value. as an example of there are 3 text fields like (store, product, name) i want to show it as below

    1) store field values as –
    store name 1 = 56 <- 56 is the other counted forms which has the same value for this field
    store name 2 = 67 <- 67 “….”
    store name 3 = 12
    sore name 4 = 30
    ..
    ..
    ..
    as added by users
