You’ll have to configure a default registration page from Global Settings -> Default Pages for the register link to appear on the login form. For lost password link, please enable the option and set the password recovery page from Login Form Dashboard -> Password Recovery settings.

Is it possible to give an ID and Name to the form fields and connect them with MySql database?
