I’m trying to test this plugin but I have a few questions.

What is the “list id”? What creates the list? How is the list used?

What is the “category”? What creates the category? How is the category used?

By adding my custom post type to Review Post Types I see the ajax box in the post page. However, I am unable to display anything using shortcode. For my use, reviews will be done after a sale and not on the post page, like Ebay. I need to use shortcode to display the form for the appropriate post ID. Is this possible?

I think pictures along with shortcode examples would be very helpful in the documentation. There are very few users and no examples of how it should look. This plugin looks very promising if I can figure out how to make the shortcode work.
