  • As far as visitor-lead conversion goes, this plugin accomplishes what you want. It would be great if the form field values could take php variables. I recommend this plugin.

    Can you give me some use cases? Allowing PHP there has security considerations so I want to understand how it might be used before considering this as a feature.
