doesn’t create a functions file

the plugin doesn’t create a functions file in my plugins folder, but it successfully edited a copy of my functions file that i placed in the folder. But it doesn’t activate when i create a plugin header in the file.
can you provide a copy of what the proper functions.php plugin file contains so i can manually create one?
