Doesn′t work with categorys applied to pages

Hi,
thank you for the plugin! I am using additional the plugin Add Category to Pages.
Now the plugin has stopped working. The subpages don′t appear or will just be displayed on the top level (not as a submenu).
Here screenshots of the settings I have tried:
