Donation form not loading

Hi
I have installed the free version of the plugin for a client, set up a form and put the short code in a widget at the bottom of the sidebar on the page shown. However, the form isn’t loading. I have tried deactivating most of the plugins and changing the theme to but that didn’t make any difference.
