Hello! I checked your form at https://arfm.org/donate. What I’m seeing is that the Siteground Optimizer plugin is compressing and combining JavaScript. What I’d like you to test is temporarily disable the Siteground Optimizer, clear any cache, and see if the form works as expected.
If it works: you’ll need to exclude some common GiveWP JavaScript files from the Siteground Optimizer plugin’s settings:
give.js
give-donation-summary.js
You may or may not have the following in your site:
give-ffm-frontend.js
give-tributes-common.min.js
give-fee-recovery-public.min.js
give-recurring.min.js
