Download not triggered

I have advanced downloads all set up on a course I am offering. I have it set up where customers can use one coupon for a $30 discount, and a second coupon for a free e-book download BOGO.

When I test it, everything looks great on the cart and through checkout. But after payment is completed, the BOGO just kind of evaporates. It doesn’t show on receipts, and there is nothing indicating to the customer how to access it. Help?
