Drag & Drop form not visible

Hello,
I’m trying to add this form to a client’s website. However, the form isn’t visible and I “think” I installed accurately. What am I doing wrong? Here is form code:

Please let me know.
I would scrap that form, make a new on. Use the buttons I mentioned earlier.
It will write the form code , then save the form. try again to test it.
Make sure you set the upload limit in the Drag & Drop form settings to be enough for the upload max you want to allow.as you can see, before I saved my form I added text MAX file upload 5mb. That will show under upload box so up loader (the person) will know the max file size.
