Drop Down Box to Switch Forms

I have a few forms:
1) General Enquiries
2) Wedding Video Enquiry
3) (not yet developed)I would like to present these on the one Contact page, with the option for the user to switch to the appropriate one. It would also be nice if this could be done with AJAX so that the page refreshed with the correct form.
Is this possible?
