Duplicate form appearing when you press submit

Hi there

I have been having issues with the form.

It does not happen all the time, but there have been a couple of instances where the form was filled in by a user and it does not send because a duplicate form is generated underneath the original form.

Can someone please tell me why this is happening? Your help is appreciated. Thanks in advance.
