Duplicate Order Customer Charged Twice Only One Order Confirmation email

Hi,
Just had a duplicate order created on a woocommerce 5.0.0 site
Customer placed one order but two orders created and 2 payments processed.
First order has no data in the “Order Notes” section where second order shows inventory adjustment, order status change and charge id for stripe payment.
Admin or customer received no email notification for the first order.
Site has been running fine for last year with no issues.
