I would suggest using Contact Form 7. That will provide you with the capabilities you are looking for.

I am looking at the plugin and it does look nice. One question is how would I go about emailing each individual contact member? Would I need to link the names to a new page for the email as in click one name go to a new page that will have the basic submit question layout?
