edit form

on this page https://lincolnhighschoolclassof197940thclassreunion.com/test/ how do I just have one field to upload images? I don’t need the other fields and I tried removing them but no matter what I delete, they still remain. I would like to remove the following fields: title, post content, title, description and one of the upload fields.
