Edit List Forms Code

Hello. I have a client who uses LeadPages and wants me to incorporate WPNewsman with it. LeadPages has an html option and worked fine BUT there are fields that I did not put on my form & are showing when I put the code in.
First this is the form I am referencing. I only ask for First Name and Email.
Here is the screenshot of what comes up when I put the code the form gives me into the html section.
Where are these bottom 2 fields coming from? If you look on the left I can delete the bottom 2 fields but when I the form doesn’t work.
Where do I go and how do I change this code??
