Email Before Download Link not showing on landing page

Hi can some help me please…I really need some assistance with the email before download, contact 7 and download manager issue.

I have the following plugins
CF7 version 4.2.2
Download Manager version 1.8.1
Email Before download version 3.4

followed instruction specified on on the Email before download wordpress plugin website

also can’t forget to mention. I have placed the above shortcode on a landing page created with plugin called landing pages. Wondering if this could be causing the issue.

Form sends email notification that form has been filled out, however no download link displays once sent
Any thoughts ?
