Email field disappears with EDD AffiliateWP Ninja Integration plugins.

I’m using EDD + AffiliateWP + Ninja Forms (and the integration plugin). I’ve used the manual method to create an affiliate registration form, and I’ve used the import data method to auto-magically create the form. In both cases, whenever I preview the form, or load it cleanly from a registration page with a short code, I get the same result. The Email field disappears. I kid you not.

Am I the first person this has happened to, does anyone have any helpful ideas how to fix this frustrating state of affairs, please…?
