Email field not showing in MailPoet form

Hello,

I seem to have an odd issue, on the site I am helping a friend update (I did not create the custom theme), she has three forms on her footer.

All three of them have a pop-up that displays an email field to subscribe. The middle one, Sign Up for Updates, is not displaying the email field.

All three of them are working correctly on her staging site, and I have compared the code in the footer between live and staging and there is no difference. Both sites are running the latest MailPoet version, even though this issue showed up before updating.
