Email Form Field and Subscribe Button Not visible

    Using the Plugin for coming soon mode at https://www.nefenduro.com
    unfortunately am unable to see the form ( email ) field and the subscribe button.
    I have configured both the email and form tab in the plugin.
    Where am I going wrong ?
    please help
