Email, Name and Event name box not appearing

I think the current theme I have has somehow broken the Email Name and Event name boxes in the submit event part of the plugin.

I disabled all plugins, and it still looked the same, and I know it was working on a different theme. I’m just not sure what is causing these boxes to not appear as the ones further down the submission form are fine.
