Email not arriving from form submission

I’ve used CHECK E-Mail plugin to verify my PHP email is working.

I receive the Auto Responder email I setup. Which indicates the plugin can send email.

I do not receive the form email…

I’ve searched and searched, but I can’t find my solution. Lots of people with issues, but not this specific of information.

I really hope this can be resolved quickly.
