Embed Username Changer with a Simple Form

I’m using Ultimate Member and would like to add this to the user account settings page or even one of the profile tabs to logged in users.

Is there a shortcode or function that I can use to display a simple Username Changer form so a user can change their username?

The user would already be registered and logged-in when they saw the form.

I would really appreciate it if someone could help out.
