Enquiry Forms not available

Hi,
We have the plugin installed but when the user clicks the enquiry button an opaque screen covers the form and stops users from clicking into the form fields.

You can see it happening on all products e.g.

I’ve tried disabling plugins but can’t seem to find the cause.

Can someone take a look?
