Error 404 when publishing from Google Forms

    I have created a form using the old version of Google Forms and added it in the Google Forms plugin.

    When I add the share link and save, I can see the page when I preview draft. As soon as I publish I am receiving the 404 page not found.
    I contacted my host, the did not see any errors generated. I tried disabling the ModSecurity. The tech at the host created a new form, with the same error. I have refreshed all cache, changed browsers, Google Chrome, Firefox, Safari.

    Changed the form back to draft mode, appears fine. Publish.. No luck! Can anyone provide any assistance?

    Not sure what happened… Turned all my plugins back on and finally, it worked.
