Error jquery with plugin

    I inserted the plugin on my site and it seems to work properly.
    Unfortunately, the graphics of the form is not clear, what am I wrong?

    Glad to help. It looks like the plugin CSS is not being loaded on the page. Please check the setting “Form Style” and make sure that you have it set to include the recommended option. Then save changes and check the results. Let me know how it goes.
