Thanks for the update. I checked the source code of the form and the error message says “: Trying to access array offset on value of type bool in line 50” but it doesn’t say which file.
That leaves us back to square one.

Check the server error logs to see if you can see the full error message.

In the meantime, you should turn off the display of those notices on production sites with something like this added to the wp-config.php file:

// Hide general PHP errors 
@ini_set('display_errors', 0);
