Error occured while awarding badge!

We’ve been using BadgeOS for a while now, but in the past few weeks whenever we go to the Submission Reviews area to Approve an achievement, we get the message “Error occurred while awarding badge!” beneath the Approve/Deny buttons. The achievement does not appear to have been awarded, but when we go to that user’s account, the achievement is in fact showing as earned. This wouldn’t be a huge deal as the user is getting credit for the achievement, but we are concerned that since the achievement was not awarded in the “normal” way, the user might not be getting an email notification that their submission was accepted. I’m sure this is something silly that I am missing on my end, but for the life of me I can’t figure out what’s going on.
