Error on submission

    I am receiving an intermittent error when submitting this form. The error is: There was a problem with your submission. Please go back and try again.

    This started after updating to version 1.6.1. I updated the plugin to the latest version (1.6.2.2) but the error remains.

    Can you look into this?
