Try enabling wp debug and wp debug log in wp-config.php file.

I had the same problem because I deleted Username in the form builder. Reinstalling the plugin and leaving the field username in the account and register form fixed my problem. The function for [#login_details#] in ‘Registration activate email’ appears not able to work without the username
