If it work with default wordpress theme, it can be not compatible with your current theme.

Yes, I tried it with several different themes, and had the same problem. Comments still did not work with default wordpress themes.

How do I find out if I have a wp_comments table? What is the code for such a table, and can I insert it into my site myself?

Will removing and reinstalling WP 4.1.1 fix this problem without wiping out all of my past blogs?

Are you familiar with MySQL with phpmyadmin?

Then by this tutorial: https://www.youtube.com/watch?v=2P_iaiw1III start from 7:17 browse your wp_comment and check is there any saved data of previous comments still in there?
If you still have the wp_comment data, you can try to reinstall wordpress by backup all your files and database first.
