Error: "This template has more than 3 display ads" – but only 3 visible

When placing ads, I get the message “This template has more than 3 display ads. The maximum allowed is 3 ads per template. Remove some before you save.” But I see only 3 green boxes. There is no option to remove all ads from that page and start over again.
