Errors that doesn’t affect the form

Why yor plugin tells me about errors and the emails works fine?

Examples:

In Reply to
[NombreMedico]<[EmailMedico]>

In additional headers
Reply-To: [EmailMedico]

Also in the first tab (Form) say: “Multiple form controls are in a single label element.” But I headed again and again the texts and were is a </label> for each <label> for only 1 form text.

I really don′t understand.

The surprise is that the contact form emails arrive perfect and I can respond to those emails.
