FORM MANAGER – Errors with Shortcode table and List Box

Hi,
there are two functions which are not working correctly for me.

1. I defined a new list with List Box Style and added four list items in it. Whenever I choosed one of the items the previous item was sent in the notification mail.
EXAMPLE: I have four list items: 250, 500, 1000 and 3000.
I choosed 30000 and the notification mail had included the 1000 value. The same happened with the 1000 and 500 values.

2. The shortcode for table view is completely ignored and not working. The page is showing me the standard output.

I’ve the latest version of form manager.
