    This plugin deserves more than 5 stars, Better yet Mr. Roland the author does. I use this plugin for my clubs website. If you have a problem, Mr. Roland puts forth ever effort to answer you with in 24 hours or less, and the best part about it, this plugin is FREE!!

    You can create a form to just about any aspect. Front end and back end editing.

    Create pages to have aspects of the data enter to be displayed on page or post, for example first name, last name, the list is endless.

    Kudos to you Mr. Roland for your continuing support and add-ons!!
