Extend form

I would like to add more fields to the registration form like e.g.:
Adress
Zip code
City
Phone number
e.g.. All fields should be mandatory and info should:
– send by mail to the admin or
– stored into the WP database

I can imagine, that someone befor did something like this. So it would be great to get some feedback or a link with some info, which files I have to change and how I can do this.
Just to let you know, I′m using clean login on a multilanguage site using WPML and running on 3 different URL′s
