Extra Badges

My users are receiving badges they have not earned.
For example, user account awaters (Amber Waters) has commented on Mobile Thing 1 on 1/12 and this awarded her Badge 1 and Badge 5 (5 should be 5 steps – commenting on Things 1-5). Both badges were triggered when she completed the first step. User Samantha Noll (iamasam28) also completed Step 1 on 1.12 but she did not receive both badges.
