Extra related posts, thumbnails broken

Hi, I’m using WordPress Related Posts in my blog, and have it configured to show 3 related posts. Themes are enabled, using Vertical (Large).

However, in certain posts (not all) the plugin is displaying more than 3 related posts, but 5. What happens is that the 3rd thumbnail has a wrong title (same as 1st), the 4th thumbnail is a duplicate of the 2nd, and the 5th has the title of the 3rd but with a broken image.
