False cookie warning at login with WP version 3.9.2

version 3.9.2 all plugins and themes up to date

ERROR: Cookies are blocked or not supported by your browser. You must enable cookies to use WordPress.

I went all through the plugins deactivating and reactivating
– Bottom line – refreshing the page almost always clears up the issue.

Others have posted code inserts as a workaround but we have found refreshing the page almost always works.

This really should be addressed in WordPress as code inserts are not an optimal solution.

Browsers we tested firefox and chrome latest versions with cookies enabled.

Theme 2012 (WP and all plugins and all themes up to date)

5 Active plugins:
Deactivating and reactivating all plugins SAME problem.
