Fanbridge settings?

Hi – I’m trying to configure news extended for use with fanbridge and am running into a problem. Their form action looks like this:

On submission it is taking me to the fan bridge sign up page versus a confirm page. Not sure what I’m doing wrong. The complete fan bridge embed code looks like this:
