I do not know how to tell why the forms are reset. They would only normally do that if you go to the plugin page in admin and click deactivate, then click delete.

Can you tell me where in the database the settings are stored? I may be new to WordPress but I know my way around SQL. I skimmed through the tables for this site and didn’t see where they’re kept but I may have just overlooked something.

in wp_options
There may be a few rows starting with “si_contact_form”
The data is serialized array, so you should not change it with table edits.
The preferred way to change the settings is by editing your forms from the admin settings pages.

I am trying to insert the plugin -fast secure contact form on my wordpress page. I went through the instructions and entered the following script on my HTML Page but it does not display it. I am using CPanel and I got the URL from there for the si-contact-form.php

The code entered on my wordpress page goes as follows:

Please let me know what changes need to be done.

The code you posted is not how to use the plugin, take that off your page.
