Fast Secure Contact Form not working

Hi,
I am using a Fast Secure Contact Form but it won’t register in the Leading dashboard. The pop up works, and I get the e-mails in my Mailchimp list, but the form signups wouldn’t show up.

but the leading implementation is done on a child theme that’s not accessible.

Any ideas how to fix that?
