Fast Secure Contact Form Re-direct Issue (NEW)

I’ve been thru all the testing: default Twenty Fourteen theme switch, deactivate all plugins except FSCF, no field named “name,” etc. Read the FAQ articles on FSCF site and posts on www.ads-software.com, StackExchange, etc.
Basic contact form works fine. Reservation form goes to 404 page on submit/re-direct. Contact also has a re-direct to a Thank You page and works fine. So far, the issue occurs in iOS devices and Windows 8.

Busy time of the year for student to sign up for courses. Gotta figure this out. Appreciate help.
